When first navigating to the Certifications tab, if you have not previously created any certifications, you will be prompted to Create Certification.
If you have already created a Certification, you can toggle between Compliance Programs and click the Create New Certification button.
1) Certification Info
You will be taken to the New Certification screen, where you can start populating the details of your certification:
- Compliance Program (Required): Select the Audit Project this certification is related to
- Certification Owner (Required): Select the Owner of the certification. This list will be populated with a list of FQ users.
- Certification Name (Required): Populate with a short description of your certification.
- Description (Optional): Populate with a long description of your certification.
- Type: Choose between creating a standard questionnaire or having a Controls review.
2) Add Questions
Next, you will be able to add the question(s) you’d like recipients to respond to:
- Select whether you want multiple-choice or short-answer responses
- Populate the details of your question with either multiple choice answer options or questions for short answer responses
- For multiple-choice questions, select which response option will result in an exception
- Click Done
- If applicable, +Add Another Question
- Use the six dot icon to the left of each question to reorder the certification questions.
3) Certification Settings
Next, you will be asked if you want to reference certain materials to share with individuals.
- Attach Control Details (Optional): have Control details displayed that are a reference for the certification.
- Attach References (Optional): Attach files as references for the certification.
4) Collect Responses
Next, define who you would like to send the certification to.

Email Invitation
- Add Recipients: Add a Recipient from your existing list of contacts or create a new contact.
- Simply click Add Recipient, and you will be taken to the Select Contacts window.
- My List: This will show you all contacts that have been added to your Contact List. (Note: By default, this list will be populated with existing FQ users).
- Create Contact: Create a new contact by entering their First name, Last name and Email address.
- Import Contacts: Bulk import a list of contacts by uploading an excel list of your contacts, including First Name, Last Name, and Email (see example of excel template below)
- My List: This will show you all contacts that have been added to your Contact List. (Note: By default, this list will be populated with existing FQ users).

Add Certification Rounds

- This will set the cadence your certification is sent. Add as many rounds as necessary, set Custom names for the rounds, and set Due dates. (Note: Users commonly set their certification rounds to quarterly).

Initial Notifications and Notification Settings

Here you can enter the number of calendar days before the first notification gets sent out as well as managing any other subsequent due and past due notifications.

Once the Certification is created, the request will be sent via email to the recipient(s) on the applicable date, as set by number of days you input under Certification Request.
