While you are completing testing in the Tests tab and your Testing Status is ‘Ineffective’ you will be prompted to create an issue. You can click on ‘Create an Issue’ in the pop-up, or manually click ‘Create an Issue” in the issues section.
This will open up a new page where you can edit all the attributes of the Issue, such as the Issue Title and Issue Description. You will not be able to edit the Audit Project, Testing Phase, or Control ID as you are creating the Issue right from the Control/Test itself.
You can then Assess your Issue by choosing the Impact (Very low, Low, Moderate, High, Critical), Issue Type (Control Deficiency, Significant Deficiency, Material Weakness), and Deficiency Type (Design Deficiency, Operational Deficiency, Design Deficiency, and Operational Deficiency).
Finally, you can add a Primary Owner and Secondary Owner to the Issue. Primary Owners are required, but Secondary Owners are optional.
